Ideal Plastic Surgery – Sacramento, California

These Terms and Conditions govern participation in the Giving Back Campaign, a promotional and charitable initiative organized by Ideal Plastic Surgery, located in Sacramento, California. By submitting a testimonial video or otherwise participating in the campaign, you agree to be bound by these Terms and Conditions.

1. SPONSOR

This campaign is organized and administered by:

 Sacramento, California, United States

References to “Ideal Plastic Surgery,” “we,” “our,” or “Sponsor” mean Ideal Plastic Surgery.

2. CAMPAIGN PURPOSE

The Giving Back Campaign is a promotional charitable initiative through which Ideal Plastic Surgery will make donations to Fundación Ideal based on eligible testimonial video submissions received from qualifying participants.

3. CAMPAIGN PERIOD

The campaign begins on [Insert Start Date] and ends on [Insert End Date], unless earlier terminated, extended, or modified at Sponsor’s sole discretion.

The campaign may also end once the total donation goal of USD $250,000 has been reached.

4. ELIGIBILITY

Participation is open only to individuals who:

1. Are legal residents of the United States
2. Are eighteen (18) years of age or older at the time of submission
3. Are current or former patients of Ideal Plastic Surgery

Employees, contractors, agents, and immediate family members of Sponsor may be excluded at Sponsor’s discretion.

Void where prohibited by law.

5. HOW TO PARTICIPATE

To participate, an eligible individual must:

1. Complete the official campaign form
2. Provide accurate contact information
3. Submit a testimonial video describing their experience with Ideal Plastic Surgery
4. Accept the required media and content authorizations

Limit of one (1) submission per person unless otherwise approved by Sponsor.

6. DONATION COMMITMENT

For each eligible testimonial video submission received and accepted under these Terms and Conditions, Ideal Plastic Surgery will donate USD $250 to Fundación Ideal.

Donations will be made directly by Sponsor to Fundación Ideal. Participants will not receive any portion of the donation funds.

The maximum total donation commitment under this campaign is USD $250,000.

Once that amount has been reached, Sponsor may close the campaign without prior notice.

7. ELIGIBLE SUBMISSIONS

To qualify, a submission must:

• Be submitted by an eligible participant
• Reflect a genuine personal experience
• Be complete and truthful
• Be in a usable digital format
• Not contain unlawful, offensive, defamatory, obscene, or misleading content
• Not infringe upon any third-party rights, privacy rights, or intellectual property rights
• Ideal Plastic Surgery reserves the sole right to determine whether a submission qualifies.

8. NO ADDITIONAL PURCHASE NECESSARY

No additional purchase is required to participate, provided the participant is already a current or former patient of Ideal Plastic Surgery.

Participation does not improve or affect medical candidacy, treatment outcomes, financing approval, scheduling priority, or future services.

9. MEDIA RELEASE AND CONTENT LICENSE

By submitting any video, image, audio, text, likeness, name, voice, or related materials, participant grants Ideal Plastic Surgery and its affiliates a perpetual, worldwide, royalty-free, transferable license to use, reproduce, edit, publish, distribute, adapt, display, and otherwise use such materials for lawful marketing, advertising, promotional, educational, and business purposes in any media now known or later developed.

Participant represents that they have the legal right to grant this authorization.

10. MEDICAL DISCLAIMER

This campaign is promotional in nature and does not constitute medical advice.

Participation does not create a physician-patient relationship, nor guarantee consultation approval, candidacy, financing, surgery scheduling, or specific results.

All medical decisions must be made through a formal consultation with licensed professionals.

11. PRIVACY

Personal information collected in connection with this campaign may be used for campaign administration, follow-up communications, marketing purposes where legally permitted, and other lawful business purposes, subject to Sponsor’s Privacy Policy and applicable law.

12. LIMITATION OF LIABILITY

To the fullest extent permitted by law, Ideal Plastic Surgery shall not be liable for:

• Late, lost, incomplete, corrupted, or misdirected submissions
• Technical failures or website interruptions
• Unauthorized access
• Errors in promotional materials

• Any indirect, incidental, or consequential damages arising from participation

13. RIGHT TO MODIFY OR CANCEL

Ideal Plastic Surgery reserves the right to suspend, modify, extend, shorten, or cancel the campaign at any time and for any reason, including fraud, technical failures, legal compliance concerns, or circumstances beyond its reasonable control.

14. GOVERNING LAW

These Terms and Conditions shall be governed by the laws of the State of California, without regard to conflict of law principles.

Any disputes arising from the campaign shall be resolved exclusively in the state or federal courts located in Sacramento County, California.

15. SEVERABILITY

If any provision of these Terms is found unenforceable, the remaining provisions shall remain in full force and effect.

16. ACCEPTANCE

By participating, you acknowledge that you have read, understood, and agreed to these Terms and Conditions.
