Surgery to remove excess skin after major weight loss. What are my options?

Losing a significant amount of weight can be a major change for your health, your body, and the way you feel about yourself. However, you should know that after significant weight loss, a new concern can appear: the one to remove excess skin and sagging. When skin has been stretched for a long time, it can lose some of its elasticity and may not always fully adapt to your body’s new contour.

As a board-certified plastic surgeon, I know that for some people, this excess skin can cause discomfort when getting dressed, during physical activity, or simply when looking in the mirror. This is where plastic surgery can become an option to remove the skin the body can no longer eliminate on its own and help improve body contour.

Why does skin remain loose after losing weight?

Skin has the ability to stretch and adapt to changes in the body. However, when weight loss is significant, especially after carrying excess weight for an extended period, skin can lose elasticity.

The amount of skin left after weight loss can vary from person to person and depends on different factors, such as the amount of weight lost, age, genetics, skin quality, and how long the skin was stretched.

Which areas of the body can have excess skin?

Excess skin can appear in different areas after major weight loss. Some of the most common areas are:

  • Abdomen: loose skin can remain in the lower abdomen.
  • Arms: sagging and excess skin can appear on the inner arm.
  • Thighs: excess skin can affect the inner or outer thighs.
  • Back: excess skin and tissue can remain around the back and sides.
  • Breasts: volume loss can be accompanied by excess skin and sagging breast tissue.

Distribution is different for every patient, which is why during the evaluation, we assess the entire body, not just one specific area.

Which procedure helps remove skin after weight loss?

There isn’t a single correct order for performing these surgeries. It depends on what bothers the patient most. For me, an Abdominoplasty tends to be one of the most satisfying surgeries because it can remove a very large amount of excess tissue and create a significant, visible change.

But other patients are more concerned about other areas, such as the arms or legs. The ideal approach is to first listen to the patient. Then, if they’re hoping to have several surgeries, we work to develop a reasonable sequence that produces meaningful improvement, keeping safety at the center of the plan.

Abdominoplasty

Abdominoplasty, or Tummy Tuck, removes excess skin and fat from the abdomen and, when indicated, repairs separated or weakened abdominal muscles. It’s one of the most well-known procedures for patients with a significant amount of excess skin after weight loss.

Arm Lift

Brachioplasty, or an Arm Lift, is designed to remove excess skin from the arms, especially when sagging is the result of significant weight loss. Depending on the amount and location of excess skin, different techniques and incisions can be used.

Thigh Lift

When there’s excess skin on the thighs, a Thigh Lift can help remove that skin and improve the contour of the legs.

Can some procedures be combined?

Yes, but combinations should be chosen carefully. Procedures performed in nearby areas can sometimes be combined efficiently. For example, breast surgery and abdominal surgery are often combined in appropriately selected patients.

Upper arm surgery can also, in some cases, be combined with removing excess skin from the upper back.

The goal isn’t to determine how much surgery we can fit into a single day. The goal is to determine what can be performed safely for that particular patient.

Ideal weight for surgery after weight loss

As a board-certified surgeon and a high-quality practice, we follow standards that guide our care, including a strict BMI limit of 40. Patients with a BMI above 40 aren’t candidates for body contouring surgery because the risk of complications increases significantly. This decision is focused solely on our patients’ safety.

The decision to move forward with surgery is personalized. We evaluate the patient’s overall health, anatomy, surgical goals, the areas to be treated, and what can reasonably be achieved without making the surgery too long or introducing unnecessary risk.

I understand that reaching a healthy weight is an achievement that can represent a major change. But it’s also normal for the body to look different after a significant transformation.

If after losing weight you notice excess skin that doesn’t improve with exercise or over time, it doesn’t mean you did anything wrong. It’s simply a consequence that can occur after major weight loss.

Plastic surgery can help you address those areas and continue your body’s transformation.

At Ideal Plastic Surgery, we can support you through different stages of this process: from managing your weight loss through Ideal Weight Loss, to evaluating body contouring procedures once you’ve reached a stable weight.

The first step is learning about your options, evaluating your anatomy, and creating a plan that makes sense for you. Schedule your consultation.
